You need to look at the situation, if the paint is holding well, then there is no need to completely remove it (wash out the entire ceiling). As for stains:
Here it is important to find out whether there are aggressive compounds in these spots? There were cases when efflorescence appeared on the ceiling, and greasy spots and other nasty things that simply cannot be covered with paint. Even 1 mm. putty sometimes did not help. That's why I say, act according to the situation.
Before painting, in any case, we first prime the spots with liquid paint, which we will paint the entire ceiling with. This will “even out” the tone of the ceiling, and remove places with increased absorbency. If this is not done, then dirt may show through in places of stains and the texture will be rougher, as the paint will absorb and dry too quickly. You will not have time to roll out with a roller.
To summarize: If the stains are "chemically inactive", that is, they do not eat into the new paint, then the base and the cobweb should not be touched. We repair locally, prime with paint and cover the entire ceiling in a couple of layers. If the spots are “problematic”, then it is necessary to clean up to concrete, impregnate with a primer and, after a good drying, restore the finishing preparation.

If the paint does not darken, then you can paint the entire ceiling. But if the stain is translucent and penetrates through fresh paint, then these stains will have to be completely cleaned and puttied and prepared for painting. Since even 5 layers of paint will not block the stain.
